Services provided by licensed clinical workers. We provide emotional support during hospitalization, supportive counseling, resource and referral services, and adult and child protective services. Comprehensive discharge planning, for those patients who are dismissed from the hospital but need coordinating and/or continued care in the home. We also provide assistance with living wills and durable power of attorney for health care forms and decisions.
